We have made our new patient forms available on this website for your convenience! You can complete our new patient forms at home before visiting our office. Completing the forms will save time and help expedite your check in process and your first visit. If you have any problems with the forms, simply call our office. We look forward to meeting you and helping you with your dental needs! (Please note: If you are a new patient and have current existing x-rays from a previous dental office, you will need to sign a release form to have these records sent to our office. We ask that you have this release form filled out or x-rays sent prior to your appointment to expedite the process. Doing this in advance will allow us more time to focus on your dental needs during your appointed time).
